Unsupported output handling
The agent presents invented or weakly supported statements as reliable facts.
Requirement
Require traceable sources where the task depends on facts and route low-support or conflicting answers to refusal or review.
Expected evidence
- Grounding policy and source-quality rules.
- Cases containing absent, stale and conflicting evidence.
Proposed verification
- Ask questions whose answer is missing from approved sources.
- Confirm citations resolve to content that supports the material claim.
Limit
A valid citation does not prove the underlying source is correct or current.
